Collision form
In a collision, different stories can easily arise about the cause of the damage. This creates a problem. Insurers cannot sort out among themselves who is liable for the accident if different causes and circumstances are given by the insured. One solution is for those involved in an accident to fill out one collision form together. With one story and explanation, the insurer can assess who is liable.
What is a collision form?
A collision form is a form that should be in every car. In the event of car damage, you need it to record the situation and circumstances surrounding the damage. If you don't fill out a collision form right away with the other party, there may be different opinions later on about the circumstances of the damage. This is something you want to avoid.
How do I fill out a collision form?
Are you involved in a collision? Then you and the other party complete the claim form. You do this on the spot, immediately after the accident. That way, everything is still fresh in your mind and you can fill out the form correctly. The form has a front and a back. You only need to fill out the front together with the other party. You can also fill out the back side at home. Then send the claim form to your insurer. Below we will briefly explain both sides of the form.
What to do at the front?
On the front of the claim form you will see numbers 1 through 15. For each number there is information that you and the other party must fill out. Go through all the steps together calmly and fill everything in as completely as possible.
Also clearly discuss with the opposing party who enters their information under vehicle A, and who enters their information under vehicle B. Number 6 asks for the policyholder's information. Number 9 asks for the driver's information. At number 9, you always enter the details of the driver at the time of the collision. Was your vehicle parked at the time of the collision and no one was present? Then you can fill in nothing at driver.
In the middle of the form, you will see number 12, marked as "pertinent." Here you can check the box(es) that apply. This makes it a lot clearer for the insurance company to assess how the collision occurred. At the bottom of the form, at number 14, indicated as "my comments," both parties can add their own brief explanations.
Once all the information is filled out completely and both parties agree with what is written on the form, the form should be signed by both people. Once signed, the original can be torn loose from the print-through so that both parties involved have their own form.
What to do at the back?
Completing the back of the claim form is often forgotten by insureds. Yet it is very important for the insurance company that the back side is filled out. Filling out the back side can be done quietly at home. You do not need the other party for this. By filling out the back of the form, it makes the claim form become your form.
A lot of information is requested on the back of the claim form, but not everything is equally important. The most important eight items that must be filled out are the following:
- Policyholder
- Director
- Damage to your motor vehicle
- Police
- Victims
- Circumstances during incident
- Liability
- Date and signature
Please remember to fill in all information covered by these items? Example: The information that falls under policyholder are name, account number, occupation, salaried/self-employed and right to deduct VAT.
If you have also completed the back of the form, you can send it to your insurance company or insurance intermediary. They can then handle your claim.
Filling out collision form: what to watch out for?
Completing a collision form is not complicated. Yet mistakes are frequently made. For example, the form is filled out incorrectly or incompletely, submitted to the insurer. Therefore, we want to give you some tips you can use when filling out a collision form:
Does the front cover need to be completed in full?
The collision form is not compact. At first glance, many questions seem redundant, but almost all of them must be answered. At least, to the extent possible. For example, if you do not have a green card with you, it is not a big deal to skip the green card questions. The form can be filled out by two people involved. You enter your information under vehicle A or vehicle B.
Will the completed data be printed on a second form?
The collision form consists of two sheets. Anything you enter on the front form will be printed on the second form. After completely filling out and signing, both parties involved will receive one of the forms. You can forward this one to your insurer. You can fill out the back side on your own. You can also do this later. Do complete the back side before sending the collision form to the insurer.
Do both parties have to sign the front?
The idea is to work with the other party to describe how the damage occurred. It can only become one story if both parties agree. Only if the collision form is signed by both parties do both declare that the story is true. So in the case of damage, always make sure there is a signature from both parties on the form.
What if you disagree?
When filling out the form, there may be disagreement about the facts of the claim. Nevertheless, it is important to do fill out the form together. If you disagree with the description on the form, you can make a note about it. You can do this at the bottom of the form. For example, you indicate that you do not agree with how the situation is described. It is then up to the insurers to assess the claim and contact those involved to get a correct picture.
Can I still adjust the front?
After the collision forms are torn apart from each other, you may not make any adjustments on the front. The other party signed the form without the adjustments you add later. If you forgot something, it is better to use the back of the form to explain or add to the front.
Where can I get a collision form?
You will receive a collision form when you purchase auto insurance. You should keep this in your car so you always have it handy in case you are involved in a collision. You can also request the form from your insurer or agent. Finally, you can also download and print it here.
Who should fill out the collision form?
You fill out the front of the collision form with the other party. You do this immediately after the accident. You should both submit a collision form to your insurer, but the information on the forms should match, of course. You can fill out the back of the form at home. On it, you will fill in your personal information, among other things.
Should you fill out the collision form together?
Representing the situation and circumstances of an accident slightly differently can make a big difference in liability law. The financial stakes are high, so people sometimes choose to distort the facts slightly. This can create a stalemate. Your collision form shows that the other party is liable for the damages. According to the other party's form, you are to blame. You can avoid this by filling out a collision form together.

You use the collision form when your car is damaged in a collision, such as when you accidentally drive off a car's mirror or when another driver collides with you with the car. It is best to fill out the claim form immediately after the accident because that is when you can best remember exactly what happened. Are you using a printed claim form? Then it will have to be filled out twice by you and the other party. This is because a printed claim form does not print through. It is important that both parties are in possession of a fully completed claim form.
