It’s best to fill out the accident report form immediately after the accident, since that’s when you’ll remember exactly what happened. It’s a good idea to fill out the front of the accident report form together with the other party. Both parties need a completed form to send to their insurance company. How do I fill out an accident report form?
If you’re involved in a collision, you and the other party should fill out the accident report form together. Do this on the spot, immediately after the accident. That way, everything is still fresh in your memory, and you can fill out the form correctly. The form has a front and a back. You only need to fill out the front side together with the other party. You can fill out the back side at home. Then, send the accident report form to your insurer. Below, we’ll briefly explain both sides of the form.
What should I do on the front page of an accident report form?
On the front of the accident report form, you will see the numbers 1 through 15. Each number corresponds to information that you and the other party must fill out together. Take your time to go through all the steps together and fill out the form as completely as possible.
Also, clearly discuss with the other party who will fill in the information for Vehicle A and who will fill in the information for Vehicle B. Item 6 asks for the policyholder’s information. Question 9 asks for the driver’s information. For question 9, you should always enter the information of the driver who was behind the wheel at the time of the collision. If your vehicle was parked at the time of the collision and no one was present, then you should leave the driver section blank.
In the middle of the form, you’ll see number 12, labeled “circumstances.” Here, you can check the box or boxes that apply. This makes it much easier for the insurance company to assess how the collision occurred. At the bottom of the form, under number 14, labeled “my comments,” both parties can provide a brief explanation of their own.
Once all the information has been filled out completely and both parties agree with the contents of the form, the form must be signed by both individuals. After signing, the original can be torn off from the carbon copy so that both parties have their own copy of the form.
What should you do on the back of an accident report form?
Policyholders often forget to fill out thebackof the accident report form. However, it is very important to the insurance company that the back be filled out. You can take your time filling out the back at home. You do not need the other party to do this. By filling out the back of the form, the accident report form becomes *your* form.
The back of the accident report form asks for a lot of information, but not all of it is equally important. The eight most important items that must be filled out are as follows:

- Director
- Damage to Your Motor Vehicle
- Police
- Victims
- Circumstances at the Time of the Incident
- Liability
- Date and Signature
Please be sure to fill in all the information covered by these items. Example: The information required for the policyholder includes name, account number, occupation, employee/self-employed status, and eligibility for a VAT deduction.
If you have also filled out the back of the form, you can send it to your insurance company or insurance broker. They can then process your claim.
